If you are a past client, please submit the form below, and we will share the $20 credit opportunity with the individual you refer.
Quick Rules:
- To earn the $20 credit each, the individual you refer must complete their Initial Assessment.
- There is a maximum of 10 referrals that you can make per month, and you potentially earn up to $200* in account credits that you can use toward your sessions!
- The individual cannot be a current client.
- The individual can book with any dietitian.
- There is no cash value or referral limit increase even if a person does not book.
- The referral credits cannot be combined with other promotions or deals when paying for a session or package. They do not expire.
- *The credit amount may change without notice, but this page will be updated. The current rate of $20 is guaranteed until January 31, 2023.

By completing this form, you understand the person you refer will be contacted within CASL / receive an email from us. For both to receive the referral reward of a $20 account credit, an Initial Assessment must be completed by the person you referred.
