Referral Steps:
STEP 1
Use a referral form and fax it to NutriProCan: 1.855.933.2309, or find us on OCEAN.
STEP 2
A team member will contact the patient to review individual counselling options as well as costs, which are often covered by benefits.
STEP 3
If the patient starts counselling, the dietitian will provide updates to the referring professional. (with patient consent)

Initial Assessment
The dietitian will learn about the patient’s past and present health, work, lifestyle, activity levels, eating habits and food preferences so they can create a fully personalized and optimized plan.

Meal Plan Follow-Up Session
Personalized, flexible meal plan, which the dietitian will review with the patient. Plus, during this session, coaching begins to help patients prioritize new routines based on their lifestyle and readiness for change. The final plan is delivered to the patient via our app for easy access on the go.
During ongoing coaching, the plan is adjusted as needed to make sure it works for the patient.

Continued Support and Coaching
A typical client starts with bi-weekly coaching sessions and often shifts to monthly sessions for 3–18 months. We recommend working with a dietitian for a full year to build sustainable habits through the seasons, holidays, and life’s changing demands. Every journey is unique and fully personalized!
